WITH EVERY SERVICE REQUEST.

1.     A letter addressed to the US Passport agency explaining your need for the second passport, please click here for a sample letter

2.     Current passport which must have at least one year of validity. If not, you will need to renew it before applying for the second passport.

3.     Two (2) recent identical passport size pictures (2”x2”) (less than 90 days old)

4.     One (1) completed and signed application form DSP-82

5.     Travel Itinerary showing departure date and travel to Israel within 5 days (for 24 hours service).

6.     An authorization Letter with original signature of applicant

7.     ABC Visa and Passport SVCS request form filled out.

8.     ABC VISA & PASSPORT service fee

9.     Return Postage fee, please click for postage rates

The government fee of $ 170.00 payable to “US Department of State”
