1. A letter addressed to the US Passport agency explaining your need for the second passport, please click here for a sample letter
   
2. Current passport must have at least one year of validity, if not, you will need to renew it before applying for the second passport.
3. Two (2) recent identical passport size pictures (2”x2”)
4. One (1) completed and signed application form DSP-82
5. Travel Itinerary showing departure date within 3 days
6. An authorization Letter with original signature of applicant
7. ABC Visa and Passport SVCS request form filled out.
8. ABC VISA & PASSPORT service fee
9. Return Postage fee, please click for postage rates

US PASSPORT AGENCY FEES:
The government fee of $ 195.00 payable to “US Department of State”
