REQUIREMENTS FOR PASSPORT RENEWAL:

 IMPORTANT:

 * IT IS MANDATORY TO INCLUDE THE ABC PASSPORT REQUEST FORM AND ABC AUTHORIZATION FORM WITH EVERY SERVICE REQUEST.

 * PLEASE NOTE THAT ALL SIGNATURES ON FORMS MUST BE ORIGINALS; FORMS WITH DIGITALLY PRINTED SIGNATURES  WILL NOT BE ACCEPTED BY THE STATE’S DEPARTMENT.

You can apply for a Passport Renewal only if:

* You have in your possession your most recent U.S. Passport
* Your most recent passport was issued not more than 15 years ago
* Your most recent passport was issued when you were 16 or older

If the requirements listed above do not apply to you please see requirements for new or First Time Passport.
(U.S. Citizens Note: If you have two (2) U.S. passports, second passports CANNOT be renewed. You must complete
 the second passport application process.)

REQUIREMENTS:

1.      Your most recent passport

2.      A completed and signed application form (DS-82)

3.      One (1) recent passport size picture (2”x2”) (less than 90 days old)

4.      A copy of the airline ticket or travel itinerary showing your departure date within three days if no visa is required or ten days if a visa is required. (for 24 hour service)

5.      An authorization letter with original signature from the applicant.

6.      ABC Visa & Passport SVCS request form

7.      If the name you are currently using differs from the name on your most recent passport, please submit an original marriage certificate or court order with the official seal of the issuing authority, indicating the change of name.
