IMPORTANT: IT IS MANDATORY TO INCLUDE THE ABC PASSPORT REQUEST FORM WITH EVERY SERVICE REQUEST.

REQUIREMENTS:

1. One completed and signed application form DS-4085
2. Your valid U.S. passport
3. An authorization letter with an original signature of the applicant.
4. Copy of travel itinerary or an air tickets showing departure date or letter from employer stating immediate international travel is required.
5. ABC passport request form

US PASSPORT AGENCY FEES:

If amendment is requested within 01 year of the passport issue date:    $  60.00 payable to “US Department of State”
If amendment is requested after 01 year from the passport issue date, applicant must apply for a passport renewal, click here for details .
