IMPORTANT: IT IS MANDATORY TO INCLUDE THE ABC PASSPORT REQUEST FORM WITH EVERY SERVICE REQUEST.

REQUIREMENTS:

1. One completed and signed application form DS-4085
2. Your valid U.S. passport
3. An authorization letter with an original signature of the applicant.
4. Copy of travel itinerary or an air tickets showing departure date or letter from employer stating immediate international travel is required.
5. ABC request form

US PASSPORT AGENCY FEES:
$ 142.00 payable to “US Department of State”
